A message to Parents . . .

about the North Carolina Child Service Coordination Program

The Child Service Coordination (CSC) Program:

• Identifies infants and children who may be at risk for developmental delays
• Provides developmental screening
• Assists and guides parents in getting helpful services
• Assists in establishing regular medical care and immunizations
• Helps families build on their strengths
• Provides support to families
• Provides education to families about caring for their children

Who is eligible?

• Children from birth to three years old who are at risk for developmental delays, disabilities, serious illnesses, social or emotional problems
• Children from birth to 5 years old who have a diagnosed developmental delay, disability, illness, social or emotional problem

How do we get referrals?

Referrals can come from a wide variety of sources. Most commonly, they come from hospitals, Departments of Social Services, doctor's offices, or developmental evaluation providers. They can also come from concerned parents, individuals, day cares, schools, and others.

What services are available through the CSC Program?

• The family will be evaluated to help identify needs
• Assistance with locating and using any helpful services
• Support and education
• Developmental evaluations

Is there a fee for Child Service Coordination?

NO! This is a FREE and voluntary program. It is available to any parent regardless of income.

How do I enroll my child?

• Sign the coordination letter of agreement
• Maintain contact with your Child Service Coordinator
• Inform your Child Service Coordinator of changes in address or phone number
• Call your Child Service Coordinator with any questions or concerns
• Make sure your child receives routine medical care -- well baby check-ups, immunizations and developmental evaluations.
